Order Confirmation Form

A document that is sent to patients via email after checkout. In order for them to take a test, they should present this at the collection site/venue. This contains the details regarding the patient’s online test purchase and appointment setting.

Things to take note of:

  1. Open the registered email you used for your Dashlabs.ai

  2. Check if you’ve received the order confirmation email

  3. Present the order confirmation form to the personnel on your scheduled appointment

If you have not received one, go back to the order confirmation page and choose any of the following options:

  • Resend Confirmation: Resend a copy of the confirmation to the registered email address

  • Download Order Confirmation: Manually download the order confirmation form to the device you are using

Order Confirmation Form

Present this to the collection site/venue on your scheduled visit. This document will contain all the information about the test you booked via the online shop as well as the summary of payment that has been made.
