Proof that the laboratory is registered and licensed to operate in order for them to be verified laboratories with access to the Dashlabs.ai software.

Compliance Document

The documents uploaded will be used to verify if the laboratory has the registration and licenses needed in order for them to operate.

Title

Encode the complete Title that appears on the laboratory’s compliance document.

Validity Date

Encode the Validity Start and End Date of when the compliance document was issued and until when it is valid.

Number

Encode the Number of the compliance document. This is the unique identifier of the laboratory’s compliance document.

Laboratory Branch

Encode the Laboratory Branch of the compliance document. This is intended to differentiate compliance documents of the laboratory.

Compliance Document Type

Choose among the options to which type the compliance document the laboratory will provide are uploading falls under:

• DTI Registration;

• SEC Registration;

• BIR Form 2303;

• Business Permit;

• License to Operate;

• SARS-COV2 License to Operate;

• XRAY License to Operate;

• OFW License to Operate;

• Drug Test License to Operate.

Upload Compliance Document

Upload a soft copy of the laboratory’s compliance document.

If the file is successfully attached, there will be a prompt at the lower left side of the screen and also a list of the uploaded file is shown.

Click “Create” after all the needed information is encoded to upload the compliance document to the software and be used to verify your laboratory.
